I have pretty poor vision. I generally use a large screen (22") and a lot of magnification.
In the Windoze world I have used ZoomText and JAWS for Windows. They both cost over $500 up front and have upgrades that cost $150+ every time Windows burps. They outran my budget a long time ago.
In the LInux world we have emacspeak and an open source speech synthesizer called Flite.
A very good LiveCD version is produced by Oralux. Though still in alpha state it is most useful.
Gnome is working on Gnopernicus that is a screen reader and magnifier. The version that comes with Debian is a little out of date.
Hence the experimentation with Gentoo.
